PATIENT FORMS

It is vitally important that all new patient forms be submitted no later than 3 days prior to your New Patient Office Visit for Dr. Patane's review. If forms are not completed by this time a portion of your initial visit will be utilized to complete and review the forms and the full visit will be cut short. Thank you in advance for your cooperation as part of this team! 

Our office utilizes a cutting edge-tool called Living Matrix for our patient interactions. This tool allows us to take a thorough and in-depth patient history that you, as the patient, provide before our first visit, and quantify it into a report containing a Functional Medicine Matrix and a Timeline that will help guide us to the root cause and triggers of your health issues. Our goal is to help discover the reasons why you are experiencing health concerns so that we may develop the most accurate plan possible, together, and this tool allows us to do just that.

We recognize that this is a time consuming process, however, it is the foundation for our first visit and we truly honor and appreciate the time you put into filling out the forms fully. 

1.) Dates are extremely important in this step. If you don't know an exact date a close guess will work but a date must be entered for our report to be accurate. 

2.) Please don't hesitate to mention ALL health concerns, even symptoms that may not appear to be connected. To us, it is all connected and we are trained to recognize patterns, even those that have been missed in the past or may seem unrelated.

3.) You can start the process and come back to finish at any point 3 days prior to our first visit by returning to this page and clicking the link. Simply enter your login information. Your input will be saved. 

4.) On the 3-Day Diet Journal, please be as honest with yourself as possible for this. There is nothing that will shock Dr. Patane and there will be absolutely no judgements or lectures. As a team, this is all part of the puzzle to get to the root of your health issues and the more honest of a representation of your daily diet the better. Ideally this could be completed with the rest of the intake forms, however it is not a necessity to have it submitted 3 days prior if time is a factor.
